Agency Membership Application Instructions 
and Information


To Become an Affiliated Governmental Agency Member:  

  • Select the level of membership from the dues chart
  • Print and complete the application
  • Attach the requested information for each individual identified to participate

Please provide all requested information:

  1. Indicate the agency member contact and the agency billing contact.
  2. Select a membership level from the dues chart which corresponds to the number of individuals included in your application.
  3. Attach your check for the current dues amount which corresponds to the selected membership level.  If you wish, we will bill your agency.  Please attach an official agency purchase order.  We also accept credit card payments (Visa, Mastercard, American Express).  Provide account number and expiration date and the name of the cardholder.  Note: If you are applying during the second half of the year, contact ITE Headquarters for payment information.  Memberships run on a calendar year and may be prorated.
  4. List the names of all members to be included in your membership.
  5. List the ITE specialty council selected by each individual. Multiple council memberships per individual are available at additional cost.
  6. Indicate if each individual is currently an ITE member. (If you are unsure of membership status, please fax a list of individuals to be included to ITE Headquarters. We will verify the status of each individual.)
  7. VERY IMPORTANT! For each individual who is not currently an ITE member, attach a chronological resume and the completed first page of an ITE membership application. (If you do not have an ITE membership application, contact ITE Headquarters.)
  8. Mail the completed application to ITE Headquarters.
